We are pleased to welcome two graduate students from Dalhousie University presenting on topics in Oral and Maxillofacial Surgery.

Dr. Amr Alsabbagh – Personalized opioid prescribing with an individualized tapering protocol following orthognathic surgery reduces the amount of unused opioid.

This prospective randomized controlled trial investigated whether an individualized postoperative opioid prescribing algorithm could reduce excess opioid dispensation following orthognathic surgery. Participants were randomized to either a conventional fixed-quantity hydromorphone prescription or a personalized regimen derived from inpatient opioid utilization and a structured tapering protocol. The individualized strategy was associated with a significant reduction in residual opioid quantity at two weeks postoperatively, without a corresponding increase in pain intensity, analgesic inadequacy, patient dissatisfaction, or postoperative morbidity. These findings support opioid prescribing models that incorporate perioperative analgesic requirements to minimize unnecessary opioid exposure and reduce the volume of unused opioids available for diversion.

Dr. Abdul El-Rabbany – The Use of Single-Dose vs. Short-Term Peri-Operative Cefazolin in the Prevention of Surgical Site Infection Following Orthognathic Surgery.

Surgical site infections remain an important complication of orthognathic surgery, yet there is an ongoing debate regarding the optimal duration of peri-operative antibiotic prophylaxis. While extended antibiotic regimens have historically been used, concerns regarding adverse effects, healthcare costs, and antimicrobial resistance have highlighted the importance of determining whether shorter regimens provide comparable protection against infection.

This presentation will provide an overview of an ongoing prospective randomized controlled trial comparing a single pre-operative dose of intravenous cefazolin with a 24-hour peri-operative cefazolin regimen in patients undergoing orthognathic surgery. The study aims to determine whether a single pre-operative dose provides similar protection against surgical site infection while reducing unnecessary antibiotic exposure.
